Disability and Life Insurance

Short-Term Disability Insurance

ESI provides its full time employees with short-term disability insurance covering loss of employee income because of non-job-related short-term disability (STD). All full time ESI employees are automatically eligible the first of the month following 30 days of full time employment. No coverage is available for spouses or dependents.

An employee must apply for STD coverage under ESI's policy with Aetna and provide medical documentation regarding the disability. If it is determined that this illness, injury or pregnancy does qualify for STD coverage, benefits will be paid at the rate of 60 percent of pre-disability weekly earnings, up to a maximum of $2,000 per week after all available sick, vacation, personal holiday and FMLA leave has been exhausted.

A 15-day elimination period is required for each disability.

Long-Term Disability Insurance

All full-time employees of ESI International who are actively at work are eligible for coverage on the first day of the month following 30 days of service. ESI offers long-term disability (LTD) benefits for loss of employee income because of non-job-related accident or sickness. An employee must be continuously disabled for 180 days before becoming eligible. No coverage is available for spouses or dependents.

LTD benefits are paid at the rate of 50 percent of pre-disability earnings including commission, with a maximum benefit of $3,000 per month.

If you elect to pay the premium on the Long Term Disability insurance, then any claims paid to you for a disability will be tax free. If you elect not to pay the premium, claims paid will be subject to income tax.

Life/AD&D

ESI provides $50,000 of life insurance and accidental death or dismemberment insurance for its full time employees. You must be actively at work to be eligible for coverage on the first day of the month following 30 days of employment.

Optional Term Life Insurance

Optional term life insurance is available to employees and their family members through Aetna. The Optional life insurance policy is portable – you can take it with you if you leave ESI.
